Post-merge cleanup for #463 CLI restructure, addressing review feedback from 小墨.

Why

Two pieces of dead code remained after the restructure merge.

Changes

thread.ts — Remove cmdThreadRunning function (CLI already uses thread list --status running)
README.md — Remove step read from docs (command not registered, half-baked)
cli-reference.ts — Update uwf skill cli output to reflect new four-layer commands
