Breaking change per review:
- Remove systemPrompt from RoleDefinition entirely
- identity/prepare/execute/report are now required (string, not nullable)
- Remove all legacy fallback logic in buildRolePrompt
- Simplify validate.ts, workflow.ts materialize
- Migrate all test fixtures and example workflows
